Systemic training issues
In modern flight training, there is often a gap between theory and practical application, which is never fully explained to the student. In many cases, critical information is given to the student only once, with the expectation that it will be understood and applied when needed. The information may (or may not) be fully absorbed and understood by newer pilots. However, applying theory correctly when it is suddenly needed in real life may not happen as intended, if at all.
